Jeffrey W. Slack, 39, of Brazil died unexpectedly Friday, Dec. 19, 2003, in the emergency room of St. Vincent Clay Hospital.
Born June 13, 1964, in Brazil, he was the son of Jimmie G. and Judith Muncie Slack.
A graduate of Brazil High School, he had worked for Hancor and Great Dane in Brazil; and for K&P Vending in Paris, Ill. He also owned and operated Act One Video in Paris for several years.
Survivors include his mother, Judy Kopchak, of Brazil; a brother, Jim Slack, and wife, Lisa, of Martinsville, Ind.; a sister, Jana Vinzant, and husband, Ed, of Brazil; two nieces, Stephanie Heavrin and Tiffany Slack; three nephews, Jeremy Kendrick, Jimmy Vinzant and Zach Vinzant; two great-nieces; a great-nephew; aunts; uncles; and other extended family.
He was preceded in death by his father.
Family and close friends will gather for services at 1 p.m. today at Lawson-Slack funeral Home. Pastor Tim Russell will officiate. Visitation will be 11 a.m.-1 p.m. prior to services. Burial will be in Clearview cemetery.
