Shila Addison Haas Carter, 4 months and 6 days old, died Friday, Oct. 1, 2004, at 11:06 p.m. in Riley Hospital at Indianapolis.

Born May 25, 2004, at Indiana University Hospital, she was premature and had Cystic Bronchial Pulmonary Disease.

Survivors include her parents, Dustin Ray Carter and Christina Marie Haas; two brothers, Keenan Yates and Dustin Carter, all of Terre Haute; grandparents, Monica and Jay Haas of Terre Haute, Cindy Carter of Terre Haute, Richard and Sharon Carter of Kankakee, Ill.; great-grandmothers Margaraet Haas and Ruth Haas; five uncles, Christopher White, Chad White, Devin Carter, Richard Carter Jr. and Ryan Carter; two aunts, Stormy Carter and Shannon Carter; and many cousins and other extended family.

Services will be Wednesday at 2 p.m. in French Funeral Home. Burial will follow in Oak Hill Cemetery.

Visitation will be noon-2 p.m.

Donations may be made in her memory to the Riley Hospital Children's Fund. Envelopes will be available at the funeral home.
