Lewis Dale Mayfield, 55 of Brazil, died at 3:35 a.m. Thursday, December 22, 2005, at Union Hospital in Terre Haute, IN.

At the request of the deceased, his body was cremated. A memorial service will be held 11 a.m. Saturday, January 7, at Bee Ridge Church on Kennedys Crossing Road.
