State seeks to put nursing home on probation

INDIANAPOLIS -- The Indiana State Department of Health has filed an administrative action against the license of Hoosier Care II, Inc., doing business as Clay County Health Center, 1408 E. Hendrix St., Brazil.

State health officials said they are seeking the facility's license be placed on probation, or in the alternative, be revoked.

State health officials are asking the probationary license include requirements for 30 hours of nurse consulting and 30 hours of administrative consulting per week.

The charges are based on findings from a survey completed by Indiana State Department of Health on Oct. 11, 2002.

The survey said the facility failed to ensure that each resident received adequate supervision and assistance devices to prevent accidents for three of five residents identified as at risk for accidents and/or falls.

The Brazil Times contacted Clay County Health Center this morning. The employee who answered the telephone said the administrator was not available, would not identify the administrator and hung up on the reporter.

Administration of the facility changed recently, when the former administrator retired.
