He was born Sept. 1, 1935, in Clay County, as the son of John and Lelia Croy Hayes.
Following graduation from high school, he served in the United States Army. Jack and his wife Claudia worked as campground managers.
Jack was a member of the Veterans of Foreign Wars, American Legion, BPOE Elks, Eagles and the Moose Lodge.
He and his wife had attended the First Presbyterian Church of Brazil.
He was preceded in death by his parents and his wife Claudia, who also died in the motor vehicle accident Monday.
He is survived by daughter Jan Marie Gorby and husband Robert, Brazil, son Brett Hayes and wife Deborah, New Port Richey, Fla., sister Beatrice Gregg, Florida, grandchildren Dalton Hayes, Austin Hayes, Joel Rainwater, Jaclyn Robertson, Jessica Robertson and Bobbie Gorby, and other extended family members.
At the request of the deceased, his body will be cremated and a memorial service will be conducted at noon, Friday, Dec. 5, 2008, at Moore Funeral Home, with Rev. Gary Scroggins officiating.
