Jack Eugene Willoughby Sr., 60, Poland, died Nov. 12, 2007, at Terre Haute Regional Hospital.

There will be visitation Thursday, Nov. 15, 2007, from 2-3 p.m., at French Funeral Home. Donations may be made to the family.
