Youth Food Delivery Program in search of help to give families school supplies

Ascension Health St Vincent Clay Hospital was unable to get forms out to families and students before the end of the 19/20 school year for help to families with school supplies for the 20/21 school year. They have made forms available to us to deliver with our food deliveries this summer, but need your help. None of us know what the school year 20/21 will look like, but they are making school supplies available to Clay County students whatever it looks like.

The deadline to apply or have forms turned is Sunday, July 5, 2020!

Student’s first and last name

Parent’s name and phone number

Gender of child

School the child attends

Grade the child attends

Pick up is Saturday, July 18, 2020 from 9 am to 1 pm at Ascension St Vincent Clay, 1206 E National Ave, Brazil, IN 47834
